 Driver’s License, Insurance, Medicare, Medicaid cards

Contact Bradley County Health Department if you have questions @ 870-226-8441

ADH will charge your Insurance Company for the cost of the vaccine.  If you do not have Insurance, or your insurance does not pay, the shot will still be no charge to you.
